You will be receiving an email over the next few days from DocuSign on behalf of email@example.com regarding changes to your payment information. Please read this email very closely as it contains important information about how to update your payment information.
In order to complete this transition, there are 3 things in the DocuSign envelope that need to be filled out:
1) The first document is a standard W-9.
2) The second document is a request for EFT payment information.
3) After filling out both forms to completion and submitting via DocuSign, please send a voided check to Yardbarker by one of the three following methods:
• Scan and send a copy via email to firstname.lastname@example.org
• Fax a copy to 510-225-2932
• Mail a copy to Yardbarker, 611 Mission Street (2nd Floor), San Francisco, CA 94105
Please complete this process by Friday, December 17th 2010 to ensure that your payment processing schedule is not interrupted. This transition is not optional; if you do not submit these forms and a voided check you will not receive future payments.
