By now, all of you have been sent the DocuSign envelopes required for completing your payment info update. If you have not received these, please email email@example.com immediately. Here are some frequent questions we have received:
- If you are not a US citizen and therefore do not pay US taxes, you have not yet been sent the correct information. You do not need to sign a W-9, you need to sign a specific W-8. We will send this documentation to you once we have it.
- If you do not have a checking account or checks, please enter information for a savings account. In lieu of a voided check, please provide a bank statement (with the amounts blacked out) or a note from your bank stating your account info.
- If you are entering information for an online checking account (ING, HSBC, etc.) please provide a bank statement (with the amounts blacked out) or a note from your bank stating your account info.
- If you are having issues entering a phone number on the EFT form, you need to enter a number without any dashes (i.e. 5554443333).
Again, send all information to firstname.lastname@example.org and fax your voided check or bank note to 510-225-2932.
Keep in mind that we need this information submitted to us by Friday, December 17th*.
*International citizens will have an extension
